How Wegovy's pricing is structured

Wegovy's savings program is simpler than some competitors' in one respect: there's no separate middle tier for "commercial insurance that doesn't cover the drug." Novo Nordisk's model is essentially two paths — a commercial-insurance-covered discount, or a flat self-pay price through NovoCare Pharmacy that anyone can use regardless of insurance status, including people whose plan excludes the drug and people on government insurance who choose to pay outside their coverage.

The commercial-insurance path applies if you have private (non-government) insurance and a valid prescription for an FDA-approved use. Enroll through NovoCare.com or by texting SAVE to 83757, take the offer to your pharmacist, and — if your plan covers Wegovy — your copay drops toward $25, capped at $100 in total monthly savings.

The self-pay path, through NovoCare Pharmacy, is available to anyone with a valid prescription: uninsured patients, patients whose commercial plan excludes Wegovy, and — notably — government-insured patients who process the purchase outside their insurance rather than through it. This is the option most people without covered benefits should be comparing against the new Medicare bridge program below, since it doesn't require giving up any other coverage to use.

This is the offer most people mean when they say "Wegovy coupon." It functions as copay assistance: it reduces what your insurer says you owe, down to a $25 floor, capped at $100 in total savings per month. If your plan's copay is already below $100 above the $25 floor, you simply pay the lower actual copay — the card doesn't add money beyond what it's discounting.

Federal Employees Health Benefits (FEHB) plans, ACA marketplace/health exchange plans, and state employee plans are explicitly treated as commercial insurance for this offer — they are not considered government healthcare programs for eligibility purposes, per Novo Nordisk's own terms, even though some are administered through public exchanges.

NovoCare Pharmacy is Novo Nordisk's own direct-to-patient pharmacy, and these self-pay prices are available with a valid prescription regardless of insurance status — no card, no coverage decision required. The pill pricing was published as in effect through August 31, 2026, after which Novo Nordisk states pricing will be updated, so treat $149 as a current, not permanent, figure.

The $199/month pen price is explicitly a limited-time offer for new patients, covering their first two monthly fills only; after that introductory window (or after December 31, 2026, whichever comes first per the published terms), the price steps up to $349/month for standard-dose pens or $399/month for the HD pen. Existing patients who are past their first two fills should expect the higher figure, not the intro price.

Because these prices update on fixed dates that Novo Nordisk has already announced, it's worth checking NovoCare's current pricing guide directly rather than relying on a screenshot or older article — pricing you saw even a few weeks ago at a different tier can genuinely be out of date.

New in 2026: the Medicare GLP-1 Bridge

Starting July 1, 2026, Medicare added a new pathway specifically for GLP-1 obesity medications: eligible Medicare patients prescribed Wegovy for a covered, FDA-approved indication can pay $50 a month through the Medicare GLP-1 Bridge program. This is a genuinely new option — Medicare has historically excluded weight-loss-indication GLP-1 coverage — and it sits outside both the standard commercial savings offer and NovoCare Pharmacy self-pay pricing described above.

Eligibility is set by the Centers for Medicare & Medicaid Services (CMS), not guaranteed simply by having a prescription or being a Medicare enrollee — talk to your prescriber about whether your specific Medicare plan and diagnosis qualify. This program does not extend the standard commercial savings offer to Medicare patients; it's a separate, Medicare-specific bridge with its own eligibility rules administered through CMS rather than Novo Nordisk directly.

Mistakes that cost money

The most common mistake is assuming the $25 price applies to anyone with insurance. It only applies if your specific plan's formulary covers Wegovy — otherwise you're routed to self-pay pricing regardless of whether you're insured at all.

A second mistake is assuming the $199/month pen price is permanent. It's explicitly a new-patient, first-two-fills-only introductory rate; the standard self-pay pen price after that window is $349/month (or $399/month for the HD pen) — budgeting around the intro price indefinitely leads to an unpleasant surprise on the third fill.

A third mistake, specific to Medicare patients, is assuming eligibility for the $50/month bridge program automatically because you're on Medicare and have a Wegovy prescription. CMS sets the eligibility criteria for that program, and it isn't a guarantee of coverage — confirm your specific situation with your prescriber and plan rather than assuming the $50 price applies.

The Wegovy pill is an oral tablet, dosed at 1.5mg or 4mg, and it's the cheapest self-pay option at $149/month. It's a newer addition to the Wegovy lineup and isn't automatically interchangeable with the injectable forms — your prescriber decides which formulation fits your treatment plan, not price alone.

The standard Wegovy pen covers the injectable dose range from 0.25mg through 2.4mg — effectively the full titration schedule most patients start on. New patients pay $199/month for their first two fills, then $349/month afterward.

The Wegovy HD pen, at 7.2mg, is the newest and highest-dose injectable option, priced at $399/month self-pay. It's intended for patients whose prescriber has moved them beyond the standard 2.4mg maintenance dose, not as a faster way to reach a high dose from the start.

Before assuming a switch between forms is just a pricing decision, confirm with your prescriber — dose equivalence between the pill and the pen isn't necessarily 1:1, and self-pay pricing shouldn't be the deciding factor in a titration or formulation change made without clinical input.
